TERMS AND CONDITIONS

Terms and Conditions for Diagnostic Dyslexia Assessment - schools. I am delighted you are considering a dyslexia assessment. The assessment takes place on a 1:1 basis.

Timing

An assessment usually takes around 3-4 hours 


Location

School – a quiet, distraction free room 


Equipment

Assessor will provide all equipment


Fee

£600


Payment 

1)£200 when an assessment date is agreed and booked in

2)£400 to be paid no later than 48 hours before assessment commences


Prior to assessment


Please ensure that a sight test has taken place in the last 18 months and a hearing test is strongly recommended.


Three questionnaires will be sent to you: 

  • one for school to complete
  • one for the parent along 
  • A visual difficulties screener


This background information forms a vital part of the assessment process. These will need to be returned at least a week prior to the assessment. 


After assessment

I can give you a brief overview via email of how the assessment went but results will take a little longer as full interpretation of the scores is necessary.


I will produce the report and outcome in around three weeks or I will notify you if it is likely to take longer. 


If required, a call to talk through the results can be arranged.


12 months following the report: test papers will be securely destroyed (an electronic copy of report will be kept securely and remains confidential)


Cancellation

The initial payment acts as a deposit as work will have taken place before the assessment date itself. 


If you cancel within 48 hours before the assessment date and another date is not scheduled, this will be retained.


Qualifications and References

·  Level 7 qualification in Teaching and Assessing Learners with SpLD (Dyslexia) 

·  I am a Member of PATOSS 

·  An up to date Assessment Practising Certificate.

·   Enhanced DBS check (with update service)

I am more than happy to share references with you if requested.


Notes

Please note that a diagnosis of dyslexia is not guaranteed. There is a strict set of criteria that is applied to making a diagnosis.


Whatever the outcome, a full assessment is extremely useful as it examines all areas of literacy learning including general underlying ability, memory, processing and the underlying phonological skills required to learn to read, spell and write accurately and fluently. 


The report will detail the child’s strengths and areas of need. Even without a diagnosis this information will be very useful to you, the parents and the learner.
